A. Accident & Sickness Claims

1) Claim Form, duly filled and signed by the claimant.
2) Copy of passport, valid visa and Emirates ID
3) Original Medical Certificate/ Reports from the treating Doctor or Hospital/Clinic. In case of Sickness, the medical report showing the onset of the medical condition.
4) Original Discharge summary from the treating Doctor or Hospital (in case of Hospitalization)
5) Original Leave Certificate from Hospital / Clinic.
6) Police Report (for Accident related cases)
7) Any other document as may be reasonably required by the company.

B. Serious Illness Claims

1. Copy of passport, valid visa and Emirates ID
2. Medical Certificate/ Reports from the treating Doctor or Hospital/Clinic.
3. Proof of Income
4. Copy of Labour Contract
5. Any other document as may be reasonably required by the Company.

C. Accidental Death Claims

1. Claim Form, duly filled and signed by the claimant.
2. Original Death Certificate (returnable immediately after verification). In case of death occurred outside the UAE, the Death Certificate should be attested by the relevant authority in the country of death, UAE Embassy in the country where the death took place and Ministry of Foreign Affairs in UAE.
3. Accident, Forensic, Medical Reports related to the death
4. Copy of the Policy report
5. Copy of passport, valid visa and Emirates ID
6. Copy of Entry and Exit stamp in the Passport (for death outside UAE).
7. Copy of Emirates ID
8. Copy of Visa Cancellation in case the death has happened out of UAE.
9. Any other document as may be reasonably required by the company.

D. Repatriation Claims (Applicable for Death in UAE only)

1. Original Death Certificate (returnable immediately after verification)
2. Copy of passport, valid visa and Emirates ID
3. Copy of Airway Bill.
4. Original Payment Receipt of Freight / Cargo Charges.
5. Original Receipts for Embalming Charges / Original Ambulance Charges Receipts / Original Coffin Charges.
6. Original Air Ticket paid bills.
7. Copy of the Air Ticket
8. Any other document as may be reasonably required by the company.
